3. The following misprints have been corrected:
"sufering" corrected to "suffering" (page 14)
"anæthetics" corrected to "anæsthetics" (page 48)
4. Other than the corrections listed above, printer's inconsistencies in spelling, punctuation and hyphenation have been retained.
