The two column table of contents for each chapter is retained. Since the contents of Chapters II & V were split by a page break, the left column of the second page followed the right column of the preceding page. These have been reorganized as they would have been had they fit on a single page.
Only the validity of index references to footnotes have been checked. A number of them, listed below, were incorrect.
