The index was originally structured rather inconsistently as a nested list, with line-breaks separating list items. In these ebook editions, semicolons have been inserted between list items, with full stop inserted at the end of each top-level entry. The original book separated key terms from page references with comma, and key terms from "see [other topic]" with semicolon; the latter have been changed to comma.
