Transcriber's notes:
- Footnotes (with anchors [A], [B], etc.) have been moved to directly below the paragraph or table to which they belong. References (with anchors [1], [2], etc.) have been moved to the end of each chapter.
- Some footnote anchors occur more than once in the text. In these cases, there is no link back to the footnote anchor, but the browser back button should be used.
